Honouring our Champions of Care

Champions of Community Care recognizes professionals who go above and beyond in service, compassion and dedication to the people they support. Across Canada, home healthcare is the thread that holds our system together, and that dedication deserves more than appreciation. It deserves to be celebrated.

The Access Care Foundation is proud to support the inaugural Recognition & Awards Gala, with one purpose: to ensure our home healthcare heroes are seen, heard and honoured.

  • Nominations can be made by patients, families, friends, colleagues, peers and employers.
  • Frontline & allied health. Nurses, support workers, therapists and frontline staff who deliver care at home.
  • Champlain / Eastern Ontario. Celebrating those who care for our community, close to home.
