Across Canada, home healthcare is the thread that holds our healthcare system together. It is what makes it possible for people to heal, recover, and age with dignity in the place that matters most to them. Our champions’ dedication deserves more than our appreciation they deserve to be celebrated.

The Access Care Foundation is proud to announce the inaugural Champions of Community Care Recognition & Awards Gala, taking place on October 15, 2026 at the Infinity Convention Centre in Ottawa. This first-of-its-kind regional celebration has one purpose: to ensure our home healthcare heroes are seen, heard, and honoured.

Nominations are open to anyone who has witnessed excellence firsthand patients, families, friends, colleagues, peers, and employers. Winners will receive a $5,000 cash prize, gift certificates from local businesses (including home builders and car dealerships), and meaningful media recognition.

Nominate a Personal or Home Support Worker, Nurse, Allied Health Professional or Frontline Administrative Staff.

What our champions receive

Our advisory panel will select the top winners. A winner from each category will receive:

Cash prize up to $5,000

A meaningful cash award recognising the difference our champions make every day.

Gift certificates

Gift certificates from local businesses, including car dealerships and home builders.

Meaningful media recognition

Public recognition that celebrates our winners and the work they do across the region.

Eligibility criteria

Who you can nominate

Nominees should primarily deliver care in a community-based homecare setting. This recognition is intended for those working outside facility-based environments such as hospitals, acute care, long-term care, retirement or group-home settings.

Eligible professions include:

Registered Nurse (RN)Registered Practical Nurse (RPN)Personal Support Worker (PSW)Home Support Worker (HSW)Occupational Therapist (OT)Physiotherapist (PT)Speech-Language Pathologist (SLP)Registered Dietitian (RD)Social WorkerFrontline Operations Staff

A nominee should also be…

  • Currently practicing and providing direct care within home or community-based settings.
  • A minimum of 12 months of active service in home and community care.
  • Holds a current licence and is registered and in good standing with the applicable regulatory body.
  • Provides care within the Champlain / Eastern Ontario region.

How do I submit a nomination?
Click any “Nominate a Champion” button to be routed to our nomination form. It only takes a few minutes to share your champion’s story.
When do nominations close?
Nominations are open until August 15, 2026. Winners will be contacted in September 2026 and presented a formal award at the gala on October 15, 2026.
Who is eligible to be nominated?
Nominees should primarily deliver care in a community-based homecare setting (not hospitals, acute care, long-term care, retirement or group homes), have at least 12 months of active service, hold a current licence in good standing, and provide care within the Champlain / Eastern Ontario region.
Which professions can be nominated?
